Person-Centered Planning
Person-centered planning creates a safe space in which participants can discuss their future dreams and visions. It includes them in the decision-making processes. For this meeting all supporters are invited and we meet in-office or virtually on Zoom.

Person-Centered Report Writing
Your Person-Centered Plan will include all of the wonderful information, details, stories, goals, dreams, resources, ideas, proposals, and supports that were shared during the PCP Meeting.

Budget & Spending Plan Creation
The Spending Plan specifies how budget funds will be used to purchase services and supports required to carry out the Individual Program Plan (IPP). We help you create this Plan.

We support clients by attending the Individual Program Plan (IPP) meeting, budget meetings, and helping to identify, and advocate, for unmet needs.
